Once you've booked your first appointment, our front-desk team will email you a brief set of intake forms. Most patients complete them in under ten minutes.
The forms cover:
- Basic contact and demographic information
- Medical history and current medications
- The condition or symptom that brought you in
- Insurance information for verification
- Standard consent and HIPAA acknowledgment
When to expect the forms
You'll receive the email within 24 hours of booking. If you don't see it, check your spam folder or call us at (530) 955-0065.
If you'd rather complete them in person
That's fine. Arrive 10–15 minutes early for your first appointment and we'll get you set up at the front desk.
